If you would like to make an appointment, please contact our office by phone at (262)681-8829. Our office facilitates communications and emergency calls and appointments are available and welcome, especially for new patients seeking help.
New Patient's coming to see one of our Independant Providers at Caledonia Clinic can print out the intake paperwork for the provider they are seeing. These can be found under the Our Practice tab:
- Click on the Caledonia Chiropractic Intake Paperwork link and print out all the pages. Fill them out and bring them on your first appointment with Dr. Tim Schneider, D.C.
- Click on the Dr.Biboso Intake Paperwork link and print out all the pages. Fill them out and bring them on your first appointment with Dr. Biboso, M.D.
- Click on the Caledonia Physical Therapy Intake Paperwork link and print out all the pages. Fill them out and bring them on your first appointment for Physical Therapy with Tim Ulrich, P.T.
Financial
We accept most insurance and will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want. Forms of payment accepted by the office are check, cash, or MasterCard/Visa.
Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
